It is our goal to help you receive the most out of your dental benefits. As a courtesy, we will file your insurance for you. As much as it is our goal to help with your insurance reimbursement, we must point out that your insurance is an agreement between you, your employer, and the insurance company. We are not a party to this agreement. Please take the time to understand your group’s dental insurance limitation, maximum, deductible, exclusions etc. We are able to provide some assistance in estimating your insurance benefit. However, your insurance company makes the final determination once treatment is completed and the claim is submitted. Therefore, any fees quoted to you by our office are estimates only, subject to change, and are not a guarantee of insurance coverage or payment.
Any deductible or estimated co-payment amount will be due at the time of treatment.
Fees not covered by insurance are your responsibility.
We understand that circumstances arise that prevent patients from keeping their reserved appointment. If you find it impossible to keep your appointment, please notify us at least 48 HOURS prior to your appointment. This courtesy makes it possible to offer your reserved appointment time to another patient in need of dental care. If no such notice is received, we reserve the right to charge a FEE OF $50 for any missed appointments. If you have any questions, please do not hesitate to ask.
